Robust information sharing is the cornerstone of ongoing patient care, review and assessment for further intervention. The remit team are keen to improve current discharge documentation content and consistency to ensure that accurate coding can be easily undertaken in the community. This will improve the accuracy of registers, clinical activity and recall procedures and result in data collectione being improved.
You may be aware that one change already implemented is the use of the 5th digit when coding CHF on discharge. This will identify if HF-REF/HF-PEF/no indicator made. This should improve the quality of data on the primary care population level and subsequently, the treatment of heart failure across traditional boundaries.
