The Hub remains open to healthcare professionals from all backgrounds and disciplines, as well as other stakeholders with an interest in heart failure. Here’s what we’re focusing on in 2024
- Website, digital media, and communications
Our website is evolving to showcase the Hub’s work, share guidance and models of care, publicise research opportunities, and keep members updated with clinical news and developments. We have added to our social media presence with new accounts on X and BlueSky.
- Patient Engagement
Since its inception the Hub has ensured that patients have been involved in the steering group as well as delivery of an annual education event for patients and families. Collaborating with third sector colleagues, and widening opportunities for patient engagement will support access to up-to-date resources and amplify their voices in shaping service priorities.
- Education
Education remains central to our aims. We will continue to deliver our successful “Ensuring Success in Heart Failure” conference. A committee dedicated to exploring and delivering other educational initiatives has been established.
- Active Workstreams
To address challenges and opportunities in the optimisation of heart failure care, our active workstreams will focus on priority areas where national collaboration can drive change. The proposed areas for 2024 include:
Collaborative Working: Developing collaboration with other specialties across primary and secondary care to improve patient outcomes. In addition, we want to promote representation of nurses and allied professionals in the work of the Hub and in heart failure care.
Data and research: Exploring how better use of data can enhance heart failure practice across Scotland and working with Public Health Scotland to support the development of a national heart failure audit. We aim to support increased patient participation in heart failure research to advance knowledge and care.
Models of Care: We aim to identify and share examples of efficient models for assessing and managing patients with heart failure and streamlining current heart failure services without compromising care quality.
